What CarePlanKit is — and is not
CarePlanKit is a free study and drafting aid for nursing students and educators. Using it does not create a nurse–patient, provider–patient, clinical, or professional relationship of any kind between you, any patient, and CarePlanKit or its authors.
Everything on this site — worked examples, the care plan builder, diagnoses, goals, interventions, and any generated output — is general educational material assembled from study examples. It is not verified for any specific patient, is not individualized clinical guidance, and may be incomplete, outdated, or inappropriate for a given situation.
Drafts must be reviewed before any use
Any care plan you build here is an unverified draft. Before it is used for anything beyond your own study, it must be reviewed, corrected, and approved by a qualified nursing instructor or a licensed clinician, and adapted to the individual patient, their current assessment, and your institution's protocols, policies, and standards of care. Clinical judgment always rests with a qualified, licensed professional — never with this website.
Do not enter patient information
Do not enter real, identifiable patient information (PHI). Use initials or fictional data only. CarePlanKit is not a clinical record system and must not be used to store or transmit protected health information.
